 $ 190.00   *** There is an additional $5.25 fee if paying by credit card ***

Sibling Discounts:
Sibling 1:   $20 discount (fee = $170)
Sibling 2+: $30 discount (fee = $160)
Sibling 2+: $40 discount (fee = $150)

This information will guide you through the registration process! 
These instructions are the same to register for Football and for Cheer.  If you need any assistance during the registration process, please contact Butch Grantland  (melbourneducks@gmail.com) or 321-960-4926.

FIRST THINGS FIRST! CREATE YOUR ACCOUNT...

1.  You must LOGIN or REGISTER for an account with us!  Those options are on the upper right of our website. 
(As you create your account, keep in mind you are creating your PARENT account, and will then add participants)
2.  Add your participants
3.  Select a program for each child to participate in
4.  Complete the checkout process.  If you wish to pay by check instead of credit or debit card, slide the payment type to "CHECK".  You will then pay the fee in person prior to equipment distribution.  Cash and Check amounts are $190.00.  Checks are made payable to "Melbourne Pop Warner".
Note:  Payments by debit/credit cards will appear on your statement as "Blue Sombrero". 
Registration Fees are non-refundable.

NEXT - FORMS TO COMPLETE & OTHER ITEMS REQUIRED:

1. Participant Contract & Waiver
2. Medical History & Physical
3. Supplemental Form

IN ADDITION TO THE FORMS ABOVE, YOU MUST PROVIDE:
1.  A copy of the participant's Birth Certificate
2.  A recent color wallet-size photo of the participant
3.  2 copies of the participant's Final Report Card for the 2019-2020 school year
     ** If you do not have a final report card (Home School or Child was not in school yet), please fill out the appropriate form in place of the                report card:
      Home school form OR Not in school form  
4.  A copy of parent/guardian Driver’s License
5.  Registration Fee payment (or online receipt of payment)

IN-PERSON PAYMENT OPTIONS:

We accept Cash or Check (payable to Melbourne Pop Warner), as well as Visa, MasterCard, American Express and Discover. 
Please note, there is a convenience fee of 2.75% added to all card purchases. 
 
NOW WHAT?
You have a few options to get us the paperwork and payment (or proof of payment):

1. You may e-mail completed documents and other requirements to: [email protected] Please ensure documents are clear. Include the participants name in the subject line.

3.  Mail your completed forms (and check payment or payment receipt - please, do not send cash) to:

GET READY FOR THE SEASON!

1.  Practice will begin on August 1, 2020 at (pending location based on field availability) from 6pm - 8pm.

2.  Equipment (Football):  We will provide your player with Pads, Helmet and Mouthpiece at an Equipment Distribution event which will be held in July.  Specific Team arrival times are TBD.  A parent and/or guardian must sign for this equipment, and all registration requirements must be fulfilled before equipment will be issued.  You will need to provide your child with an undershirt, a practice jersey, practice pants, and cleats for practice.  Prior to the first game, we will provide a game jersey, and game pants  (game pants are not permitted to be worn during practice).
